FEES AND INSURANCE
I am an in-network provider for Medicare only.
I am an out-of-network provider for other insurance companies. If you would like to use your out-of-network insurance benefits, you will be provided with a receipt of services that you may submit to your insurance company for partial reimbursement. Please check your coverage carefully by asking the following questions:
- Do I have out-of-network outpatient mental health benefits?
- What is the out-of-network deductible I must satisfy prior to receiving reimbursement?
- How many sessions per year does my health insurance cover?
- Is approval required from my primary care physician?

Please kindly notify me at least 24 hours in advance if you are unable to attend your scheduled appointment.
